Contained here are forms used to request use of the facilities or calendar the use of the minibus (which holds 14 plus the driver) for church events. Forms are available in both Word and PDF versions. Download and complete the form and either email (chbc@clairmonthills.org) or fax (404-634-6062) to the office.
Use the "Core Ministry" forms for church events. Use "Non-Core" forms to request the use of the facility for outside events.
